Job Summary
Manage patient discharges with patient families and the health care team, developing home care plans for post-hospital needs. Serve as a resource and liaison with hospitals, skilled nursing homes, and physician offices to arrange medical equipment delivery and process discharges. Conduct daily rounds on nursing units to review discharge plan needs, complete progress notes, and monitor admissions for home care assistance. Supervise assigned personnel by directing activities, addressing complaints, and coaching staff on performance expectations. Participate in recruitment, selection, and training sessions while compiling monthly statistics and reporting assessments to the physician. Evaluate community referrals for intake support and assist in developing strategic plans for the home care program.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
