Job Summary
Manage patient discharges by consulting with patients, families, and healthcare teams to develop post-hospital care plans. Serve as a liaison with hospitals, skilled nursing facilities, and physician offices while arranging home delivery of medical equipment and supplies. Conduct daily rounds on nursing units to review discharge plan needs, monitor admissions, and complete progress notes on patient charts. Supervise assigned personnel through performance management, training sessions, and staff scheduling while addressing complaints and coaching underperforming staff. Compile monthly statistics and report patient assessments to the physician and Team Leader. Participate in recruitment, selection, and in-service education for department staff.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
- All required vaccinations as a condition of employment and annually thereafter, where applicable
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
