Job Summary
Manage patient discharges by collaborating with patients, families, and the healthcare team to develop post-hospital care plans. Serve as a resource and liaison with hospitals, skilled nursing homes, and physician offices while arranging home delivery of medical equipment and supplies. Conduct daily rounds on nursing units to review discharge needs, complete progress notes, and monitor admissions for home care assistance. Supervise assigned personnel through performance management, staff scheduling, and training sessions. Assist with recruitment, selection, and orientation of new employees while providing in-service education to hospital and medical staff. Compile monthly statistics and report patient assessments to the physician and team leader. Evaluate referrals from community agencies and advise on developing strategic plans for the home care program. Order and arrange delivery of necessary medical supplies and equipment.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
- All required vaccinations as a condition of employment and annually thereafter, where applicable
- Acknowledgement of E-Verify Participation and Right to Work notices
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
