Job Summary
Manage patient discharges by consulting with patients, families, and healthcare teams to develop post-hospital care plans. Conduct daily rounds on nursing units to review discharge needs, complete progress notes, and monitor admissions for home care assistance. Supervise assigned personnel through recruitment, training, scheduling, performance management, and periodic evaluations. Assess referrals, order medical equipment delivery, and compile monthly statistics for Home Health and hospice standards. Provide in-service education to hospital and medical staff while addressing staff complaints and performance gaps.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
