Job Summary
Manage patient discharges by coordinating with patients, families, and the healthcare team to develop post-hospital care plans. Serve as a liaison with hospitals, skilled nursing homes, and physician offices while arranging delivery of medical equipment and supplies. Conduct daily rounds on nursing units to review discharge needs, complete progress notes, and monitor admissions for home care referrals. Supervise assigned personnel through performance management, staff scheduling, and training sessions while addressing complaints and coaching underperforming staff. Compile monthly statistics and provide field chart information adhering to Home Health and hospice standards of care.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- All required vaccinations as a condition of employment
