Manage patient discharges by developing home care plans for post-hospital needs and serving as a liaison with hospitals, skilled nursing homes, and physician offices. Conduct daily rounds on nursing units to review discharge plan needs, monitor admissions, and complete progress notes on patient charts. Assess referrals for intake support, order medical equipment delivery, and provide in-service education to hospital and medical staff. Support department leadership with recruitment, training sessions, staff scheduling, and performance management while addressing staff complaints and coaching underperforming team members.
