Job Summary
Manage patient discharges by consulting with patients, families, and healthcare teams to develop post-hospital care plans. Serve as a liaison with hospitals, skilled nursing homes, and physician offices while arranging home delivery of medical equipment. Conduct daily rounds on nursing units to review discharge needs, monitor admissions, and complete progress notes. Supervise assigned personnel by directing activities, addressing complaints, and coaching staff on performance expectations. Assist with recruitment, selection, training, and orientation of new employees. Compile monthly statistics and report patient assessments to clinical leadership.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL)
- Must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
- All required vaccinations as a condition of employment
