Job Summary
Clinical Manager (RN) oversees home health services, providing in-home supervisory visits, conducting performance evaluations and disciplinary actions for assigned staff, monitoring caseloads and team productivity, coordinating interdisciplinary care, maintaining patient tracking and financial forecasting, and generating reports on census and staffing for administration. Requires RN license, graduation from an accredited nursing program, 2 years RN experience in community/home health, 1–2 years supervising clinicians in a Home Health Agency, EMR experience, CPR certification, reliable transportation, and knowledge of Medicare Conditions of Participation and home health payment mechanisms. The role emphasizes leadership, staff development, and ensuring quality of care for homebound patients.
Required Qualifications
- Licensed Registered Nurse (RN) in the state of operation
- Graduated from an accredited school of nursing
- Two (2) years of RN experience in community/home health
- One to two (1-2) years of supervision experience in a Home Health Agency
- Experience using Electronic Medical Records (EMR)
- CPR certification
- Access to reliable transportation
- Strong knowledge of Medicare Conditions of Participation and payment mechanisms for home health
