Job Summary
Assure ongoing interdisciplinary patient assessments and supervise paraprofessional and licensed professional team members to ensure clinicians meet safe, effective, and efficient standards. Use clinical decision-making to manage assigned patient populations, document complaints and infection logs, and conduct OASIS and chart audits. Identify opportunities to improve care quality, formulate action plans, and evaluate results while educating clinicians on documentation standards. Participate in QAPI programs as requested and stay current on care delivery trends. Maintain a 1:1 patient ratio with flexible assignments. Full-time Monday–Friday schedule.
Required Qualifications
- Active Registered Nurse (RN) license in the state of Arizona
- BSN preferred
- Three (3) years of nursing experience in home healthcare preferred
- Two (2) years of management experience preferred
- Two (2) years of OASIS experience preferred
- CPR Certification (demonstration course required)
- Active Arizona Finger Print Card
- Clinical proficiency in nursing specialty
- Strong communication skills
- Compassion
- Works well in a team setting
- Self-starter
- Knowledge of State and Federal home care regulations
- Computer proficient
