Job Summary
Clinical Manager/Field Nurse coordinating hospice and home health care in the Coachella Valley and Moreno Valley. Responsible for coordinating patient care, supervising clinical personnel (LPNs, HHAs), reviewing case information, assigning personnel, developing and implementing individualized plans of care, and guiding clinicians to deliver quality home care services. Involves home visits, case conferences, education for patients and caregivers, discharge planning, and collaboration with an interdisciplinary team to meet regulatory standards and improve outcomes. Requires RN licensure, management experience, and strong communication skills; driving to patient homes is required.
Required Qualifications
- Registered nurse with current licensure to practice professional nursing in the state
- Two years management or supervisory experience
- Complies with accepted professional standards and practice
Desired Qualifications
- BSN degree (preferred)
- RN license in state of practice
- Two years of management or supervisory experience
- Excellent communication and interdisciplinary collaboration
- CPR certification
- Driver with insured vehicle
- Home health/hospice experience
