Job Summary
Build and maintain relationships with hospital discharge planners, physicians, and social workers to drive referrals and represent the agency at community events. Conduct initial client consultations and home visits to assess needs, then collaborate with the care team to develop individualized care plans and support transitions from hospital or skilled nursing settings. Handle administrative functions including answering inquiries, maintaining accurate records, coordinating caregiver-client matching, and assisting with operational reporting. Requires an RN, LPN, LCSW, or related degree; full-time or part-time schedule with local travel.

- Licensed Clinical Social Worker (LCSW)
