Job Summary
Evaluate, coordinate, and facilitate referrals for acute inpatient rehabilitation by analyzing market trends, rounding on call points, and building relationships with physicians, case managers, and insurance providers. Execute strategic outreach plans to drive census growth, complete pre-admission screens compliant with CMS requirements, and collaborate with acute care staff to determine admission plans. Provide patient and family education, monitor customer satisfaction, and report activity trends to leadership while maintaining credentialing certifications across facilities. Work with the Access team to share information efficiently and solve problems with admitting physicians and referral sources.
Required Qualifications
- Active degree in nursing, social work, physical therapy, occupational therapy, athletic trainer or speech therapy
- Applicable state licensure
- Current Basic Life Support (BLS) certification
- Minimum of two years clinical experience in inpatient rehab, acute care, or post-acute setting
- Proficient in clinical documentation in electronic medical record platforms
- Strong interpersonal, organizational and communication skills
- Valid driver's license with no restrictions
- Must be willing to travel
- Working an occasional weekend/holiday
Desired Qualifications
- Previous experience working in a liaison, care management, clinical outreach or other access roles
- Experience working in inpatient rehab, LTACH, or SNF/SAR referral processes
- Understanding of CMS guidelines and insurance authorization practices
- Demonstrated ability to analyze territory metrics and referral data
- Strong presentation skills and comfort speaking in front of key referral sources
- Able to respond to highly variable workload demands
