Job Summary
Coordinate admissions and ensure timely transitions from acute to post-acute care by evaluating patient referrals and assisting with clinical screenings. Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers to build trust with healthcare partners. Serve as a clinical resource to families and referral sources while collaborating with internal facility teams to align care plans. Track referral activity and participate in census development initiatives. This regional role covers Alaska Gardens and Heartwood Extended Care in Tacoma, WA, with a starting salary of $110K.
Required Qualifications
- Active LPN or RN license (preferred)
- clinical background in healthcare, case management, or marketing
- Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred
- Excellent communication, organization, and relationship-building skills
- Ability to work independently and travel regularly between facilities
- Must reside within commuting distance of Tacoma, WA
