Job Summary
Build and maintain strong relationships with hospitals, physicians, discharge planners, and referral partners across the Billings and Laurel markets. Evaluate referrals, conduct clinical screenings and pre-admission assessments, and coordinate admissions to ensure seamless patient transitions into skilled nursing communities. Serve as a trusted clinical resource for patients and families while partnering with facility leadership to meet census and care goals. Track referral activity and participate in strategic business development initiatives. Requires an active LPN or RN license, with a preference for clinical backgrounds in skilled nursing, post-acute care, or discharge planning. Regular travel throughout the territory is necessary, and candidates must reside within a reasonable commuting distance.

- Willingness to travel regularly throughout the Billings and Laurel, Montana markets
- Must reside within a reasonable commuting distance of the territory
Desired Qualifications
- Active LPN or RN license preferred
- Clinical background in skilled nursing, post-acute care, hospital case management, discharge planning, or healthcare marketing strongly preferred
- Excellent communication, organization, and relationship-building skills
- Self-motivated with the ability to work independently across multiple locations
