Clinical Liaison

Job Summary

Assess potential residents and guests by reviewing medical records, communicating patient needs with families and physicians, and determining admission eligibility in collaboration with interdisciplinary team members. Build and maintain relationships with discharge planners, senior living providers, home health agencies, and other healthcare partners to generate activity and secure placements. Actively communicate with prospective clients via telephone, facility visits, and tours to convey community messages and educational resources regarding program utilization. Ensure accurate transmission of admission information to the Admissions Coordinator while managing payer types and obtaining current medication and treatment data.

Required Qualifications

  • Associates degree
  • Knowledge of the information and techniques needed to assess and treat human injuries and diseases
  • Effective verbal and written communication skills
  • Strong presentation skills
  • Interpersonal skills
  • Excellent customer service skills
  • Knowledge of regulatory standards, compliance requirements, and hospitals policies and procedures
  • Ability to manage territory and accounts
  • Patient care experience
  • Clean Driving Record
  • Valid driver's license
  • Visual abilities intact
  • Auditory abilities intact

Desired Qualifications

  • Previous discharge planning experience
  • Sales/Marketing experience
  • RN, LPN, RRT or paramedic
