Job Summary
Coordinate and arrange home care services between Providence Home Services and contracted facilities for home health, home infusion, and home medical equipment associated with clinical referrals. Manage care logistics for patients requiring medical equipment, ensuring seamless service delivery across Oregon. Maintain Oregon Registered Nurse licensure and obtain National Provider BLS certification within 60 days of hire. Requires one year of full-time clinical experience and strong communication skills to support patient-focused, whole-person care.
Required Qualifications
- Education sufficient for licensure
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- 1 year of Full-time experience in clinical role
