Clinical Liaison (RN, OT, PT, SLP) (FT)

Coordinate between internal and external referral sources and the acute inpatient rehabilitation operations to ensure timely patient placement. Establish relationships with case coordinators, physicians, payors, and other referral sources; conduct clinical screenings and assessments in collaboration with hospital Case Coordination and the Inpatient Rehabilitation Medical Director; work with the inpatient rehabilitation admissions team to ensure a responsive and smooth referral and admission process. Requires a current clinical license and a valid driver’s license. Inpatient rehabilitation experience is preferred.
