Clinical Liaison (RN, LPN, OT, COTA, PT, PTA, SLP, RRT) (FT)

Clinical Liaison coordinates between internal and external referral sources to ensure appropriate patient placement through clinical screenings and assessments in collaboration with hospital Case Coordination and the Inpatient Rehabilitation Medical Director. Works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to maintain a responsive, smooth referral and admission process; liaises with physicians, payors, case coordinators, and other referral sources to support timely patient placement and admission.
