Clinical Liaison Registered Nurse - Hospice

Hospice Clinical Liaison RN to support hospital-based referrals. Perform hospice eligibility assessments and bedside clinical evaluations in hospital settings; facilitate hospice admissions and complete required documentation; collaborate with physicians, case managers, social workers, and discharge planners; coordinate transitions from hospital to hospice services; educate hospital staff and referral sources on hospice services and admission process; build and maintain strong relationships with hospital partners; support urgent and GIP hospice referrals as needed. Requires active Illinois RN license, hospital or hospice experience preferred, with strong communication and organizational skills; training provided for nurses transitioning into hospice liaison work. Schedule includes weekdays with no nights, some weekend coverage, and travel within the south side of Chicago and surrounding south suburbs.
