Job Summary
Hospice Clinical Liaison RN positions require hospital-based referrals support, hospice eligibility assessments, and coordination of hospital-to-hospice transitions. The role involves collaborating with physicians, case managers, social workers, and discharge planners; educating hospital staff and referral sources on hospice services and admissions; and ensuring timely hospice referrals and admissions with a patient- and family-centered approach. Training is provided for nurses transitioning from acute care to hospice liaison work. Schedule options typically avoid nights, with occasional weekend coverage every 6-8 weeks; travel across the south side of Chicago and surrounding south suburbs is required. Preferred experience includes ICU, med-surg, oncology, ED, or case management; hospice or palliative care experience is a plus.
Required Qualifications
- Active Registered Nurse (RN) license in Illinois
- Graduate of an accredited nursing program
- 1-2+ years of RN experience (hospital or hospice experience preferred)
- Current CPR certification
- Valid driver’s license and insurance
