Clinical Liaison PRN (RN, LPN, OT, PT, SLP)

Clinical Liaison coordinates referrals and admissions for acute inpatient rehabilitation, acting as a liaison between internal teams and external sources. Builds relationships with case coordinators, physicians, payors, and other referral sources; ensures appropriate patient placement through clinical screenings and assessments in collaboration with the hospital’s Case Coordination and Inpatient Rehabilitation Medical Director; works with Case Coordinators and the admissions team to support a smooth referral/admission process. The role emphasizes strong verbal and written communication, organizational and time-management skills, attention to detail, knowledge of medical terminology, and a collaborative, patient-centered approach.
