Clinical Liaison PRN

Clinical Liaison coordinates between internal and external referral sources and the acute inpatient rehabilitation operations. Establishes relationships with case coordinators, physicians, payors, and other referral sources. Ensures appropriate patient placement through clinical screenings and assessments in collaboration with hospital Case Coordination and the Inpatient Rehabilitation Medical Director. Works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to ensure a responsive and smooth flowing referral and admission process.
