Clinical Liaison

Build and maintain strong professional relationships with medical professionals, hospitals, and healthcare organizations to support effective care coordination and referrals. Promote the organization's philosophy and objectives internally and externally while attending facility meetings, calls, and in-services to foster collaboration. Demonstrate honesty and integrity in all interactions, ensuring respectful handling of patient information and property. Maintain accurate referral-tracking logs and prepare timely end-of-day reports for department leaders to ensure operational alignment. Collaborate closely with interdisciplinary teams to support smooth transitions of care and address resident needs. Requires a valid driver's license, current RN/LPN/LSW licensure preferred, and an Associate's degree. Full-time role with same-day pay ranging from $30 to $60/hour.
