Job Summary
Clinical Liaison coordinating patient admissions and referrals for post-acute care across the Kansas City area. Builds and maintains relationships with hospitals, physicians, case managers, and community organizations; reviews referrals and coordinates admissions processes including assessments, documentation, and authorizations; conducts facility presentations and tours for referral sources and families; communicates resident needs to facility teams to ensure smooth transitions; promotes the facilities in the community and maintains knowledge of Medicare/Medicaid/Managed Care processes; travels between facilities to support referral development and admissions growth.
Required Qualifications
- 2+ years of admissions, business development, clinical liaison, or related healthcare experience
- Experience in skilled nursing, rehabilitation, post-acute care, or hospital settings preferred
- Strong communication, organizational, and relationship-building skills
- Professional customer service and phone skills
- Knowledge of Managed Care processes preferred
- LPN license preferred but not required
- Reliable transportation and willingness to travel throughout the Kansas City area
