Job Summary
Act as an advocate for patients within an acute care setting until they are safely discharged to their homes. Generate referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. Ensure a seamless transition of care throughout the discharge process by guiding patients and families through their options and verifying eligibility for care. Valid RN license in Pennsylvania and 3+ years of experience in a clinical care coordinator or liaison role required. Flu vaccination or approved exemption necessary. Required Qualifications
- Valid RN license in the state of Pennsylvania
- Valid Driver's License
- Reliable vehicle
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
Desired Qualifications
- 3+ years of experience in a clinical care coordinator, liaison, or case manager role
- Self-motivated
- Strong time management skills
- Strong communication skills
- Clinical knowledge of disease entities common to Home Health and Hospice
