Clinical Liaison

Act as an advocate for patients within an acute care setting until they are safely discharged to their homes. Generate referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. Ensure a seamless transition of care throughout the discharge process by guiding patients and families through their options and verifying eligibility for care. Valid RN license in Pennsylvania and 3+ years of experience in a clinical care coordinator or liaison role required. Flu vaccination or approved exemption necessary.
