Clinical Liaison

The Clinical Liaison coordinates referrals between internal and external sources for inpatient rehabilitation, establishing relationships with case coordinators, physicians, payors, and other referral sources. Ensures appropriate patient placement through clinical screenings and assessments in collaboration with hospital Case Coordination and the Inpatient Rehabilitation Medical Director. Works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to ensure a responsive, smooth referral and admission process.
