Job Summary
Develop patient-centered transition plans by integrating evidence-based clinical guidelines, preventative protocols, and metrics to promote quality and efficiency in post-acute care delivery. Represent Enhabit's home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups to secure executive-level opportunities. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation to safely navigate all weather conditions while driving to client sites. Requires a graduate of an approved nursing, therapy, or social work school with two years of field experience and licensure in the state of practice.
Required Qualifications
- Must be a graduate of an approved school of nursing, therapy or social work
- Must be licensed in the state where they currently practice
- Must have two (2) years of demonstrated field experience
- Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
Desired Qualifications
- A Registered Nurse RN, Physical Therapist PT or Licensed Medical Social Worker LMSW
- Three (3) years of field experience
- Previous experience in home health or healthcare sales
