Job Summary
Manage hospital referrals by reviewing clinical appropriateness and ensuring prompt responses. Work with the Admission Director to establish patient eligibility and coordinate facility transitions for smooth hospital-to-nursing home moves. Provide bedside sales and frequent hospital rounding while building and maintaining strong relationships with referral sources. Requires one year of skilled nursing experience and strong clinical and marketing skills. Located in Tolland, Connecticut at Woodlake at Tolland.
Required Qualifications
