Clinical Liaison

Clinical Liaison for Exalt Health (acute rehabilitation hospital) acts as the primary liaison between the inpatient rehab program and referral sources (hospitals, SNFs, physicians, case managers). Responsibilities include educating the community on rehab services, identifying suitable patients for transfer, and facilitating admissions. Key duties involve building and maintaining relationships with case managers, physicians, therapists, social workers, and other healthcare professionals; conducting pre-admission assessments at referral sources; coordinating transfer processes including insurance verification; ensuring timely admissions; participating in community outreach, health fairs, and marketing with the Business Development team; daily travel within a designated geographic territory to visit referral sources; developing marketing materials; tracking referrals and admissions; and providing regular reports to the Director of Business Development. Requirements include active licenses in one of the listed clinical fields, valid driver’s license, ability to work flexible hours including some evenings/weekends, strong organizational and communication skills, and proficiency with Microsoft Office.
