Clinical Liaison (CL)

Build and maintain referral relationships to drive appropriate admissions to the acute rehab unit. Assist and coordinate the intake and pre-admission screening process, performing on-site clinical judgment to determine patient appropriateness. Communicate rehabilitation options to patients and families while assessing their expectations and requirements. Secure information on patients' resources and benefits, functioning as the initial contact for external case management and discharge planners with payors and facilities. Spend approximately 80% of the day in an assigned territory. Requires RN, PT, OT, or SLP licensure with a bachelor's degree and previous clinical experience.
