Clinical Liaison (CL)

Build and maintain referral relationships to drive appropriate admissions to the acute rehab unit. Assist and coordinate the intake and pre-admission screening process while performing on-site clinical judgment to determine patient appropriateness. Communicate rehabilitation options to patients and families, assess expectations, and secure information regarding resources and benefits. Function as the initial contact for external case management and discharge planners with payors and hospital facilities. This full-time role at Henry Ford Macomb - Oakland Hospital offers a $5,000 sign-on bonus. Candidates require clinical licensure as an RN, PT, OT, SLP, or RRT with a bachelor's degree, plus previous clinical experience and EMR skills.
