Job Summary
Build and maintain relationships with hospitals, case managers, physicians, and healthcare providers to coordinate referrals and facilitate smooth transitions of care. Represent Allaire Health Services' skilled nursing and rehabilitation centers with professionalism while working closely with facility leadership to support census growth and community outreach. Travel throughout the assigned territory in Central Vermont, including secondary areas as needed, to drive patient referrals and exceptional care transitions. Valid driver's license and reliable transportation required for this full-time role.
