Clinical Leader- RN Nurse Manager Home Care

Visit geriatric clients in Mashpee, MA and surrounding areas to conduct assessments. Develop and execute client care plans while coordinating communications between clients, field staff, office, and clinicians. Supervise and support Home Health Aide staff in the field, including facilitating new hire orientation and training new employees and in-services. This role is part of the Assistive Care State Programs team, soon relocating to Mashpee.
