Clinical Leader Patient Services leads patient transfer coordination from outside hospitals and urgent care centers, connecting physicians to maintain optimized referrals; collaborates with physicians and nursing staff to ensure timely coordination of transfer and bed management; assists with transferring patients to higher or lower levels of care; serves as the contact for bed management and ensures throughput on units and adherence to transfer processes; builds collaborative relationships with physicians and interdepartmental teams to resolve issues promptly; supports the Transfer Center and hospital goals and seeks improvements aligned with departmental and organizational strategy; demonstrates professional growth through ongoing education and dissemination of knowledge to team members.
