Clinical Lead /Registered Nurse (RN)

Job Summary
Conduct comprehensive initial and reassessment nursing evaluations, develop individualized Plans of Care, and manage medication administration, wound care, and infusion therapy in the home setting. Supervise clinical staff through orientation, competency assessments, and ongoing coaching while coordinating interdisciplinary care and participating in case conferences. Ensure regulatory compliance with the California Nurse Practice Act, Title 22, and Medicare requirements by performing internal audits and supporting survey readiness activities. Lead daily clinical operations, including staffing, workload balancing, and policy input, while serving as a liaison between physicians, patients, and office leadership. Requires current unrestricted CA RN license, one year of home health experience, and ability to work weekends, nights, and holidays.
Required Qualifications
- Current unrestricted CA RN license
- Certified CPR
- Negative TB skin test or chest x-ray
- Minimum one (1) year of documented experience in a home health agency, primary care clinic or health facility
- Exceptional nursing assessment skills
- Analytic/Critical thinker
- Licensed driver with valid automobile insurance
- Ability to work on weekends, nights and holidays
- Reliable transportation
- Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull
- Must be able to lift up to 50 pounds
