Job Summary
Clinical Intake Coordinator, LPN to facilitate Home Health & Hospice referrals, determine information needs, communicate acceptance/decline of referrals, prioritize/tier hospice referrals, ensure regulatory compliance, with on-site 3–4 days/week and remote 1–2 days/week (rotating). On-call rotating; hybrid work arrangement in MO and KS; requires LPN license and relevant registrations.
Required Qualifications
- LPN License (MO and KS as applicable)
- 2 years applicable experience
- Family Care Safety Registry - Missouri Department of Health & Senior Services
- NURSYS licensure information for MO/KS
