Clinical Intake Coordinator

Receives and accepts all daily patient referrals through phone, fax, and electronic solutions, ensuring compliance with state and regulatory requirements. Establishes positive relationships with referral sources while documenting physician orders and monitoring pending referrals to assure timely patient admissions. Maintains comprehensive knowledge of community resources to assist sources in accessing services and builds community perceptions as a high-quality provider. Requires an active LPN or RN license in Massachusetts with hospice or home health experience preferred. Full-time, in-person role at the Lakeville office with weekly pay ranging from $37 to $40 per hour.
