Job Summary

Clinical Infusion Pharmacist IV provides patient-centered pharmaceutical care including infusion-related therapy, education, and clinical decision support for chemotherapy/biotherapy. Responsibilities include ensuring safety in compounding and dispensing chemotherapy, direct patient care, staff and patient education, and participation in clinical decision making. Supports policy implementation to ensure safety during preparation, administration, and monitoring of infusion drugs; adheres to regulatory standards (ASHP, NABP, TJC, DHS, DEA, USP, FDA). Precepts pharmacy residents and students; participates in training and educational activities; collaborates with physicians, nurses, and other health care providers; demonstrates strong clinical judgment, pharmacotherapy knowledge, and effective communication. Proficiency with EHR/EPIC, medication safety, inventory management, and quality improvement; ability to work independently and within a team; strong time management and multitasking abilities.

Required Qualifications

  • Doctor of Pharmacy degree
  • Pharmacist license in California (California State Board of Pharmacy) OR registered Pharmacist (RPH) in California
  • Minimum 5 years of recent relevant experience
