Job Summary
Clinical House Manager responsible for the overall operation and clinical oversight of a 6-bed Adult Foster Care home serving individuals with I/DD, ASD, and mental health diagnoses. Leads a direct-care team, ensures AFC licensing and CMH policy compliance, conducts targeted case management, coordinates MDT meetings, develops IPOS, performs LOCUS assessments, maintains clinical documentation, and oversees medication management. Manages residential operations, supports ADLs and behavioral interventions, drives resident independence and community integration, facilitates access to community resources and transportation, and ensures readiness for audits and inspections while fostering a person-centered, high-quality care environment.
Required Qualifications
- Licensed Social Worker (LLMSW or LMSW) in the State of Michigan
- Experience working with individuals with I/DD, ASD, and/or mental illness
- Knowledge of AFC licensing rules and CMH requirements
- Experience in case management and person-centered planning
- Proven ability to connect individuals to community and vocational resources
