Job Summary
Establish and maintain relationships with case managers, physicians, therapists, and social workers at referral sources like acute care hospitals and skilled nursing facilities. Conduct pre-admission assessments on-site to evaluate patient suitability for rehabilitation, review medical records, and educate patients and families about program benefits. Coordinate the transfer process including insurance verification and interdisciplinary team communication, while tracking referrals, admissions, and reporting key metrics to the Director of Business Development. Daily travel is required within a designated geographic territory to visit facilities and participate in community events such as health fairs. Flexible hours, including evenings and weekends, may be necessary to accommodate meetings with healthcare professionals.
Required Qualifications
- Valid driver's license
- Clean motor vehicle record
- Applicable insurance as required by state law
- Ability to work flexible hours, including some evenings and weekends
- Organizes and prioritizes tasks and projects, accurately completing job responsibilities within the specified time constraints
- Excellent attention to detail and accuracy
- Analytical and problem-solving abilities
- Ability to work under deadlines and handle multiple tasks
- Speaks and communicates English effectively in verbal and written format
- Writes legibly
- Reading and comprehension at the level necessary to perform job duties appropriately
- Functions proficiently in a Microsoft Windows environment (Outlook, Word, Excel, PowerPoint)
Desired Qualifications
- Licensed Vocational/Practical Nurse, Respiratory Therapist, Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) with an active license
- Minimum of 2-3 years of clinical experience in an acute care or rehabilitation setting
