Job Summary
Hospice RN PRN providing in-home hospice care in Oak Lawn, IL. Delivers nursing care, conducts initial and ongoing patient/family assessments, develops and revises care plans, administers medications, and collaborates with the interdisciplinary team to meet patient needs in home and community settings. Requires active RN licensure in Illinois with CPR certification; 1–2 years each of acute care and home care experience; Bachelor’s degree preferred. Travel to patient homes is required; per diem part-time position with duties including patient education, care coordination, and support for patients and families.
Required Qualifications
- Licensed RN in the state
- Prescribed CPR certification
- 1-2 years recent acute care in institutional setting
- 1-2 years recent home care experience
- Bachelor’s degree, hospice care experience preferred
