Job Summary
PRN Hospice RN plans, organizes and directs hospice care, conducts comprehensive patient and family assessments, develops and revises care plans, administers medications and treatments, and coordinates with the interdisciplinary team to provide in-home hospice services. Responsibilities include health assessment, nursing diagnoses, patient and family education, supporting activities of daily living, communicating with physicians, and ensuring effective care delivery in home and community settings. Requires nursing licensure in Illinois with CPR certification; 1–2 years of acute care and 1–2 years of home care experience preferred; ability to travel to patient homes is implied by the on-the-road work location; per diem/part-time scheduling.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- one (1) to two (2) years of recent experience in home care
- Current licensure in State and CPR certification
- Bachelor’s degree, with one (1) year of hospice care experience preferred
