Job Summary
Conduct comprehensive physical assessments and initiate plans of care for patients and families in home hospice settings. Re-evaluate patient status regularly, revise care plans as needs change, and administer prescribed medications and treatments. Counsel families, provide health care instructions, and assist with activities of daily living while communicating updates to physicians. Participate in interdisciplinary group meetings and collaborate with caregivers to meet emotional needs. Requires one to two years of acute care and home hospice experience, current licensure, and ability to walk, stand, and lift patients. Licensed driver with insured vehicle required. Full-time role with dental, vision, and health insurance benefits.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- One (1) to two (2) years of recent experience with home hospice care
- Current licensure in State
- CPR certification
- Bachelor's degree
- One (1) year of hospice care experience preferred
- Excellent observation skills
- Excellent verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Prolonged or considerable walking or standing
- Able to lift, position and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping bending, kneeling and/or crouching
- Visual acuity and hearing to perform required nursing skills
- Must be a licensed driver
- Automobile that is insured in accordance with state/or organization requirements
- Automobile in good working order
