Job Summary
Conduct clinical triage and high-risk outreach to assess member health concerns, direct care to appropriate resources, and support care planning for Rightway members. Engage members in clinical conversations to understand care needs, address barriers to preventative access, and coordinate appointments and referrals to close care gaps. Provide personalized condition education, medication adherence support, and care coordination while serving as a trusted health advocate throughout the member's journey. Maintain accurate documentation of assessments and guidance provided in alignment with RN scope of practice and organizational protocols.
Required Qualifications
- RN or BSN degree
- Holds an active RN license in a compact state and resides in a compact state
- 5+ years of direct patient care experience required
- Population health, disease management, or similar experience required
Desired Qualifications
- Multiple RN state licenses desired: CA, NY, MI, MA
- 5+ years of direct patient care experience, preferably in family medicine
- Triage nursing experience desired
- Care management or chronic condition support experience is desired
