Job Summary
Clinical Field Supervisor oversees patient services personnel and case managers, coordinates care services, ensures compliance with accreditation and regulatory guidelines, performs case management as needed, conducts supervisory visits for Home Health Aides, develops Plans of Care after admission, maintains patient records, and participates in performance improvement and staff training while ensuring confidentiality of patient information.
Required Qualifications
- Bachelor’s degree in nursing from an accredited nursing program
- current RN licensure in Kentucky
- at least two years of nursing practice experience in home care
- one year supervisory experience
- CPR certification
- TB skin test or willingness to receive TB test
