Clinical Field Supervisor- Bardstown

Direct supervision of patient services personnel and case managers; coordinates all care services and can perform case management functions if needed. Provides leadership and oversight to ensure compassionate, respectful care and comprehensive case management; maintains records; conducts supervisory visits for Home Health Aide patients; conducts admissions assessments and develops Plans of Care; communicates patient responses to treatments to physicians; reevaluates care needs and initiates recertification; trains and counsels staff and families; participates in performance improvement and regulatory compliance initiatives; ensures confidentiality of patient records; reports to the Director of Patient Care Services. Education and/or Experience include a Bachelor’s degree in nursing and Kentucky RN licensure with at least two years of nursing practice in home care and one year supervisory experience. CPR certification and TB skin test or willingness to receive. Location is Bardstown, KY.
