Clinical Field Supervisor directed supervision of patient services personnel and case managers; coordinates care services, performs case management as needed, ensures compliance with accreditation, federal and state regulations, and agency policies; provides leadership to nursing staff, conducts Home Health Aide visits biweekly, develops written Plans of Care after admission, reports patient responses to treatments to physicians, and supports performance improvement and record management; maintains patient confidentiality and participates in training and education requirements.
