Job Summary
Conduct thorough clinical evaluations for patients admitted to skilled nursing and rehabilitation programs. Develop and implement individualized care plans in collaboration with healthcare teams, monitor patient progress, and adjust care plans as needed. Provide medical direction and guidance to nursing staff, participate in quality improvement initiatives, and collaborate with interdisciplinary teams to ensure seamless patient care. Maintain accurate and up-to-date records and documentation. This multi-facility per diem role involves two weekends a month, some holiday coverage, and potential coverage for full-time CE vacations.
