Clinical Evaluator

Job Summary
Assess nursing needs of patients and develop individualized care plans while conducting thorough evaluations of medical histories, mental and physical status, and rehabilitation potential. Collaborate with healthcare teams to ensure comprehensive care and planning, then maintain accurate records of assessments, care plans, and progress. Provide education and support to patients, families, and caregivers. This per diem role requires a Registered Nurse or Licensed Practical Nurse licensed in Connecticut with at least three years of acute care or long-term care experience, plus two years in marketing, admissions, or clinical evaluation. Work remotely from home with flexibility to cover weekends, holidays, and varying shifts as needed.
Required Qualifications
- Registered Nurse or Licensed Practical Nurse licensed in the State of Connecticut
- at least three (3) years' experience in an acute care setting or Long Term Care experience
- Minimum of two (2) years marketing, admission, and/or clinical evaluator experience
- Interest in the nursing needs of the chronically and/or acutely ill, with knowledge of the affect on the elderly
- Ability to read, write, and comprehend English
- ability to follow oral and written instructions
- Strong written, verbal, analytical, and interpersonal skills
- Proficiency with MS Office suite (e.g., Outlook, Word, Excel)
- Ability to work hours as scheduled based on the requirements of the position assignment
- Flexibility to work weekends, holidays, and varying shifts as needed
