Job Summary
The Clinical Education & Care Transitions Coordinator is responsible for coordinating clinical aspects of resident admissions and discharges, overseeing nursing staff education and competency development, and ensuring regulatory compliance. The role includes developing and implementing staff education programs, coordinating the admission and discharge processes, conducting routine audits, and maintaining clinical competency while providing nursing care as needed. The ideal candidate must possess a valid RN license, BLS CPR certification, and have knowledge of relevant nursing practices and regulations.
Required Qualifications
- Current, unencumbered, active license to practice as a RN in this State
- BLS CPR Certification
- Preferred: 1-3 years' experience in geriatrics or post-acute care
- Preferred: 1-3 years of teaching experience
